Description
Job Purpose:
Operates in all clinic areas and in-patient areas to conduct echocardiogram (TTE and TEE), stress testing (treadmill, Dobutamine and thallium / Sestamibi with the nuclear imaging), and 24 hour holter (ECG) and BP monitoring procedures. Performs on patients of both sexes and all nationalities, thus ethical and cultural sensitivities are of paramount importance in the way the work is done. Coordinates with the cardiologists and specialists for supervision and reports.
Key Responsibilities And Duties
- Echocardiograms Transthoracic Echocardiogram ( TTE ):
- Performs 2D echo Doppler studies on adult, pediatric and neonatal patients and writes down the preliminary results.
- Notifies cardiologists of suspected abnormalities necessitating immediate attention (e.g. Pericardial tamponade, cardiac tumors, malfunctioning valve prosthesis and complex congenital heart disease, etc.).
- Prepares reports / videos of patients for official reading of the Consultant Cardiologist.
- Maintains documentation and down-time equipment used in the Echo lab.
- Maintains statistics, patient record and area monthly report.
- Reports to supervisor / department head any staff problems / malfunction of the echo equipment.
- Requests and obtains outpatients charts on all scheduled patients.
- Maintains supplies as needed for the examination room.
- Maintains daily the examination room in a clean and orderly environment for patient care (including changing the bed sheets).
- Maintains daily census record of Echo procedures completed.
- Trans-esophageal Echocardiogram ( TEE ) if applicable:
- Performs pre-procedure equipment preparation and sterilization and acquisition of signed consent form.
- Acquires quick history and physical examination about the nature of the disease.
- Helps the patient to understand what to expect, alleviate anxiety and enhance compliance during TEE examination. Also a brief drug and allergy history or dysphagia or esophageal disease should be elicited.
- line to administer sedation or emergency medications.
- Advises patient to remove dentures and places the mouth piece.
- Instructs the patient to lie on his left lateral position with neck and knee bent forward and swallowing during intubations with nasal breathing.
- Be alert in monitoring vital signs and complications.
- Monitor s the patient after the procedure at least 1-2 hours for potential complication of the procedures, anesthetic or sedative.
- Maintains census record of the procedure completed.
- Stress Test:
- Treadmill Exercise Test, Dobutamine Stress Test, Exercise Sestamibi and Presenting Sestamibi.
- Prior to the test, explains the procedure to the patient:
- to dress comfortably ( e.g. loose fitting, light weight clothing and sports shoes
- not to eat a heavy meal 4-8 hours before the test
- Asks the patient to sign a consent form as per policy.
- Explains the procedure in detail and re-assure the patient if he/she is anxious.
- Garment/s above the waist should be removed temporarily in order to attach 10 disposable electrodes.
- Prepares the skin area where the electrodes are to be attached like shaving the area if necessary, rubbing the skin vigorously with alcohol swabs.
- Connects the lead wires to the patient’s electrodes (lead wires are color coded). Position the cable yoke so that the cable does not interrupt the patient when he/she is on the treadmill.
- Records ECG preferably with the patient lying and standing.
- Takes the patient’s blood pressure, preferably with the patient lying and standing.
- Sets the maximum heart rate to be reached for the patient in question.
- Ensures the crash cart is ready for use should an emergency arise.
- Maintains daily census record of the procedure completed.
- Holter Monitoring:
- Explains the test to the patient.
- Asks the patient to remove clothing above the waist.
- Skin preparation: shaves hair if necessary, rub the electrodes sites several times with alcohol swabs.
- Attaches the electrodes for a two channel recording as shown in the operator’s manual. Fix the electrodes with adhesive plaster.
- Attaches the cable lead wires following color coding as shown in the operator’s manual. Makes stress loops to prevent from pulling the electrodes.
- After 24 hours, switch off the monitor and disconnect the patient cable lead wires from the electrodes. Take off and discard the electrodes.
- Process the unit in the computer by typing all the necessary information about the patient, scanning and saving the ECG tracings recorded for 24 hours then to be ready for analyzing of the specialist / consultant cardiologist.
- Maintains daily census record of the procedure completed.
- Ambulatory Blood Pressure Monitoring:
- Explains the test to the patient and its indication.
- Sets the machine to check blood pressure every 15-30 minutes during the day and every 1-2 hours during sleeping time.
- After 24 hours, disconnect from the patient and scan it in the computer.
- Maintains daily census record of procedure completed.
- Educates patients and their families about health conditions, treatments and preventative measures if applicable.

Experience:
Education
Minimum of 3 years of practical experience
BS Medical Technology or any related medical field
Language
English and Arabic preferred.
Licenses / Certifications
License for practice as per the regional health regulatory authority e.g. (SCFHS / DHA), ACLS and BLS

المسؤوليات والواجبات الرئيسية
- تخطيط صدى القلب عبر الصدر ( TTE ):
- إجراء دراسات إيكو دوبر ثنائية الأبعاد للمرضى البالغين والأطفال وحديثي الولادة وكتابة النتائج الأولية.
- إبلاغ أطباء القلب بأي تشوهات مشتبه بها تتطلب عناية فورية (مثل الانصباب التأموري/اندكاك العضلة القلبية، وأورام القلب، وسوء أداء الصمامات الاصطناعية، وأمراض القلب الخلقية المعقدة، وغيرها).
- إعداد التقارير/مقاطع الفيديو للمرضى للقراءة الرسمية من قبل استشاري أمراض القلب.
- صيانة التوثيق والمعدات المتوقفة عن العمل المستخدمة في مختبر الإيكو.
- الحفاظ على الإحصائيات وسجلات المرضى والتقرير الشهرية للقسم.
- رفع تقارير للمشرف / رئيس القسم عن أي مشكلات خاصة بالموظفين / أعطال في أجهزة الإيكو.
- طلب والحصول على ملفات المرضى الخارجيين لجميع المرضى المجدولين.
- توفير المستلزمات والاحتياجات لغرفة الفحص حسب الحاجة.
- المحافظة اليومية على غرفة الفحص في بيئة نظيفة ومنظمة لرعاية المرضى (بما في ذلك تغيير أغطية الأسرة).
- الاحتفاظ بسجل إحصائي يومي لإجراءات الإيكو المكتملة.
- تخطيط صدى القلب عبر المريء ( TEE ) إن أمكن:
- إجراء تحضير وتنعيم/تعقيم المعدات قبل الإجراء والحصول على نموذج الإقرار بالموافقة الموقّع.
- أخذ تاريخ مرضي سريع وإجراء فحص بدني لمعرفة طبيعة المرض.
- مساعدة المريض على فهم ما يمكن توقعه، وتخفيف القلق، وتعزيز التعاون أثناء فحص TEE. كما يجب أخذ تاريخ مختصر عن الأدوية والحساسية أو صعوبة البلع أو أمراض المريء.
- تركيب خط وريدي لإعطاء المهدئات أو أدوية الطوارئ.
- نصح المريض بإزالة طقم الأسنان وتركيب واقي الفم.
- إرشاد المريض بالاستلقاء على جانبه الأيسر مع انحناء الرقبة والركبة للأمام والبلع أثناء إدخال الأنبوب مع التنفس عبر الأنف.
- الأنتباه والمراقبة اليقظة للعلامات الحيوية والمضاعفات.
- مراقبة المريض بعد الإجراء لمدة ساعة إلى ساعتين على الأقل لملاحظة أي مضاعفات محتملة جراء الإجراء أو التخدير أو المهدئ.
- الاحتفاظ بسجل إحصائي للإجراءات المكتملة.
- اختبار الإجهاد:
- اختبار الإجهاد على جهاز المشي، اختبار إجهاد الدوبوتامين، السيستاميبي مع ممارسة الرياضة والسيستاميبي المباشر.
- قبل الاختبار، شرح الإجراء للمريض:
- ارتداء ملابس مريحة (مثل الملابس الفضفاضة والخفيفة والأحذية الرياضية)
- عدم تناول وجبة ثقيلة قبل الاختبار بـ 4-8 ساعات
- طلب التوقيع على نموذج الموافقة من المريض وفقًا للسياسة المتبعة.
- شرح الإجراء بالتفصيل وطمأنة المريض إذا كان يشعر بالقلق.
- يجب خلع الملابس فوق الخصر مؤقتًا لتثبيت 10 أقطاب كهربائية مخصصة للاستخدام المرة الواحدة.
- تحضير منطقة الجلد التي سيتم تثبيت الأقطاب الكهربائية عليها مثل حلاقة المنطقة إذا لزم الأمر، وفرك الجلد بقوة بمسحات كحولية.
- توصيل أسلاك التوصيل بأقطاب المريض (أسلاك التوصيل ترمَّز بالألوان). ضبط موضع مجمع الكابلات بحيث لا يعيق الكابل المريض أثناء وجوده على جهاز المشي.
- تسجيل رسم القلب الكهربائي (ECG) ويفضل أن يكون المريض مستلقيًا وواقفًا.
- قياس ضغط دم المريض، ويفضل في حالتي الاستلقاء والوقوف.
- تحديد الحد الأقصى لمعدل ضربات القلب المراد الوصول إليه للمريض المعني.
- التأكد من جاهزية عربة الطوارئ للاستخدام في حال حدوث أي طارئ.
- الاحتفاظ بسجل إحصائي يومي للإجراءات المكتملة.
- مراقبة الهولتر (Holter):
- شرح الفحص للمريض.
- طلب خلع الملابس فوق الخصر من المريض.
- تجهيز الجلد: حلاقة الشعر إذا لزم الأمر، وفرك مواضع الأقطاب الكهربائية عدة مرات بمسحات كحولية.
- تثبيت الأقطاب الكهربائية للتسجيل عبر قناتين كما هو موضح في دليل المشغل. تثبيت الأقطاب الكهربائية بشريط لاصق طبي.
- توصيل أسلاك كابل التوصيل مع اتباع الترميز اللوني كما هو موضح في دليل المشغل. عمل حلقات لتخفيف الشد لمنع سحب الأقطاب.
- بعد 24 ساعة، إيقاف تشغيل جهاز المراقبة وفصل أسلاك التوصيل الخاصة بالمريض عن الأقطاب الكهربائية. نزع الأقطاب والتخلص منها.
- معالجة بيانات الوحدة في الكمبيوتر عن طريق كتابة جميع المعلومات اللازمة عن المريض، ومسح وحفظ تخطيط القلب المسجل لمدة 24 ساعة لتكون جاهزة للتحليل من قبل الأخصائي / استشاري أمراض القلب.
- الاحتفاظ بسجل إحصائي يومي للإجراءات المكتملة.
- مراقبة ضغط الدم المتنقل:
- شرح الفحص والدواعي الطبية له للمريض.
- ضبط الجهاز لقياس ضغط الدم كل 15-30 دقيقة خلال النهار وكل 1-2 ساعة خلال وقت النوم.
- بعد 24 ساعة، فصل الجهاز عن المريض وقراءة/نقل البيانات إلى الكمبيوتر.
- الاحتفاظ بسجل إحصائي يومي للإجراءات المكتملة.
- تثقيف المرضى وعائلاتهم حول الحالات الصحية والعلاجات والتدابير الوقائية إن أمكن.

- أداء جميع الواجبات والمهام بطريقة تدعم قيم الرعاية المتمحورة حول الفرد في فقيه.
- المساهمة بفاعلية في مبادرات التحسين المستمر في حدود نطاق الدور الوظيفي.
- الالتزام ببروتوكولات السلامة والسعي الاستباقي لمعالجة أي مخاوف متعلقة بالسلامة في العمل.
- الحفاظ على السرية التامة لجميع المعلومات الحساسة التي يتم الاطلاع عليها أو مواجهتها أثناء العمل.
- الامتثال لسياسات ومعايير الأمن السيبراني لحماية أنظمة فقيه والمشاركة في التدريبات والمبادرات التوعوية لمنع التهديدات السيبرانية.
- الالتزام بقواعد السلوك والسياسات والمعايير الأخلاقية لرعاية فقيه ودعمها.
- إكمال التعليم والتدريب الإجباري وفقًا للمتطلبات، قبل شهر واحد على الأقل من تاريخ الانتهاء.

بكالوريوس في التقنية الطبية أو أي مجال طبي ذي صلة
